The doctors and their offenses include:

Action: License revocation
Effective Date: November 14, 1994 Nature of Misconduct: The Review Board sustained the Hearing Committee’s determination that the physician had been convicted of grand larceny and Medicaid fraud. The Review Board overruled the suspension of the physician’s license and ordered his license revoked.
